Transcript Request Form. This form must be completed for each college or university you’ve attended.

STEP 1 OF 5 - APPLICATION FOR ADMISSIONS

The US Department of Education requires colleges to disclose information for financial aid eligible programs that prepare students for gainful employment in a recognized occupation. At the Chancellor University, these programs include undergraduate programs, certificates and graduate programs. CU is pleased to provide information median loan debt and job placement rates.
